๐ Diastolic Control โ Actionable Plan
Date: Aug 26, 2026 ยท For Alin (82yo) ยท Target DIA: 67โ79 mmHg
Current Status: DIA avg 78.5 (last week) โ consistently above target. Multiple readings >80.
โก Quick Wins (This Week)
1. Sodium Hunt โ Eliminate Top Sources
- Laksa dinner Aug 20 โ confirmed trigger (DIA jumped 81-82 that day)
- Action: Ban processed/packaged Jain foods โ check labels for "sodium", "MSG", "yeast extract"
- Replace: Fresh herbs (basil, cilantro, mint, lemongrass, ginger) for flavor instead of salt/soy
- Soy sauce swap: Coconut aminos (lower sodium) or dilute regular soy 1:1 with water
- Target: <2g sodium/day โ expect 3-5mmHg DIA reduction in 2-4 weeks
2. Potassium Boost โ Synergistic with Telmisartan
- Already eating 2 bananas/day (~800-1000mg K) โ good
- Add: Spinach, mushrooms, coconut milk, tomatoes, potatoes
- Best Jain sources: Spinach (palak), mushrooms, coconut milk, tomatoes, potatoes
- Target: 3.5-4.7g potassium/day โ K + ARB = synergistic vasodilation
3. Morning Reading Protocol
- Problem: Taking 3 readings in rapid succession โ inflated DIA
- Fix: Take 1 reading after 5 min seated rest. If 2nd needed, wait 2 min between
- This alone may reduce measured DIA by 2-4mmHg (artifact correction)
๐ Lifestyle (2-4 Week Timeline)
4. Gentle Daily Walking
- 15-20 min post-meal walk (not fasted โ she's 82)
- Best: after lunch (helps both BP and digestion)
- Effect: 4-6mmHg DIA reduction over 4 weeks
- Low risk, high compliance for Jain lifestyle
5. Breathing Practice โ Direct Sympathetic Reduction
- 4-7-8 breathing: Inhale 4s, hold 7s, exhale 8s. 5 cycles, twice daily
- Reduces sympathetic tone โ direct peripheral vasodilation
- Effect: 3-5mmHg acute DIA drop with regular practice
- Can do while watching TV โ zero barrier
6. Sleep Position
- If she sleeps on right side โ may affect arterial pressure
- Try: Left-side sleeping or supine for 1 week โ recheck morning readings
- Small change, worth testing
๐ Medical (Discuss with Dr Chitra)
7. Telmisartan Timing Check
- On 40mg nightly (increased Aug 13)
- Question: Is evening dosing optimal for her chronotype?
- Morning hypertension (her pattern) may respond better to morning dosing
- Action: Ask Dr Chitra at Sep 9 review about switching to AM dosing
8. CCB Addition (If No Improvement by Sep 9)
- Telmisartan + Amlodipine 2.5-5mg = synergistic for DIA
- CCBs are most effective monotherapy for isolated diastolic HTN
- Low dose start โ monitor for ankle edema
๐ Monitoring
9. Track DIA Response to Each Intervention
- Week of sodium reduction โ compare to baseline
- Week of walking โ compare
- Don't change everything at once โ one intervention per week to isolate effect
10. Sep 9 Reminder
- Already scheduled to re-evaluate: avg DIA post-Telmisartan 40mg
- If still >80 โ escalate to Dr Chitra for CCB addition
๐ซ What to Avoid:
NSAIDs (ibuprofen, naproxen) โ raise DIA significantly
Excessive coffee โ already flagged as crash trigger (Jun 22)
Over-exercising โ 82yo, keep it gentle
Cold exposure โ vasoconstriction โ higher DIA
๐ฏ Expected Timeline
| Intervention |
Onset |
Expected DIA Reduction |
| Sodium restriction | 1-2 weeks | 3-5 mmHg |
| Potassium increase | 1 week | 2-3 mmHg |
| Walking program | 4 weeks | 4-6 mmHg |
| Breathing practice | Acute + chronic | 3-5 mmHg |
| Telmisartan full effect | 2-4 weeks | 2-4 mmHg |
Combined potential: 8-15mmHg DIA reduction โ that should bring her into range.
